Effective Date: September 30, 2026
This policy explains what maritimept.com collects when you use it, why we collect it, who else touches it, and how to ask us to change or delete it. It describes the website only. It is not a notice of privacy practices for your medical records, and this website is not a secure channel for health information. Maritime Physical Therapy is referred to below as "we," "us," or "our."
The main form on this site is the appointment request form. It asks for your name, your phone number, your email address, and a short message describing what is going on or what you want to ask us. It also has a checkbox where you can consent to receive text messages from us. Our job listings have a separate application form that asks for your name, phone number, email address, the position you are applying for, and a resume file you choose to upload. You control every field: if you do not want to give us something, leave it out and call the clinic instead. Separately, the company that hosts this website records ordinary server information in the course of delivering pages to your browser, such as your IP address, your browser and device type, and which pages were requested. We also use analytics tools, described below, to understand how visitors use the site. We do not attempt to connect that server or analytics information to a named individual.
We use what you send for one purpose: to respond to the appointment request or question you submitted. In practice that means calling or emailing you back, confirming a time, answering what you asked, and following up if we could not reach you the first time. We do not sell your information, we do not rent it, and we do not hand it to advertisers. We keep form submissions only as long as we reasonably need them to serve you and to keep our own business records.
Please keep the message field general. Naming the body part that hurts and what you are hoping to get back to is exactly right. Do not send diagnoses, imaging or test results, medication lists, insurance identification numbers, or anything else you would consider protected health information through this website or by email to us. Web forms and ordinary email are not secure, and we cannot treat them as if they were. If we need clinical detail in order to help you, we will collect it at the clinic or over the phone, where it is handled under the rules that govern patient records.
We send text messages only to people who checked the SMS consent box on the appointment request form. If you leave that box unchecked, we will still use your phone number to call you about your request, and we will not text you. Messages we do send relate to your appointment or your inquiry. Message and data rates may apply. Reply STOP to any message and the texting stops. You can also email or call the clinic and ask to be removed from any list. Opting out of texts never affects your ability to book or receive care; we will simply reach you by phone or email instead.
Some pages on this site embed content from other companies: a Google Maps frame showing our Manomet location, and a widget that displays our Google reviews. When a page containing embedded content loads, your browser connects directly to that provider, which means the provider may receive your IP address and the address of the page you are viewing, and may set its own cookies in your browser. That activity is governed by the provider's own privacy policy, not by ours, and we do not control what they collect. Most browsers and content blockers can be configured to block embedded content if you would rather it not load.
We use Google Analytics, through the Google tag, and Microsoft Clarity to understand how people find and use this website, such as which pages are visited, how long visitors stay, which buttons are clicked, and whether a form was submitted. Microsoft Clarity also records how visitors scroll, move and click on a page so we can find parts of the site that are confusing or broken. These tools use cookies and similar technologies, and they collect information such as your IP address, device and browser type, and the pages you view. Google and Microsoft process that information under their own privacy policies, available at policies.google.com/privacy and privacy.microsoft.com. You can block or delete cookies in your browser settings, and Google offers an opt-out browser add-on at tools.google.com/dlpage/gaoptout. Our site works without these cookies. We do not sell the information these tools collect.
When you submit a form, the submission is received and processed by the company that hosts this website, then passed into the contact-management system the clinic uses to track appointment requests and reply to them. Analytics information is processed by Google and Microsoft as described above. Those vendors process the information on our behalf, under their own security practices and terms, so that we can get back to you. They are not permitted to use your information for their own purposes. No vendor arrangement makes a public website a secure place to send health information, which is why we ask you not to.
You can ask what we hold from a website form submission, ask us to correct it, or ask us to delete it. Email maritimephysicaltherapy@gmail.com or call (508) 682-4217 and tell us what you would like done, and we will act on reasonable requests promptly. One thing to be clear about: deleting a website form submission is not the same as deleting a patient record. If you have been treated at the clinic, your clinical record is kept and released according to the laws that govern medical records, and you should ask the front desk about that separately.
If we change this policy we will update the effective date at the top of the page. Questions about this policy, or about how your information was handled, can go to Maritime Physical Therapy at maritimephysicaltherapy@gmail.com, by phone at (508) 682-4217, or by mail at 715 State Rd, Plymouth, MA 02360.
