What is vestibular therapy?

Dizziness that arrives when you roll over in bed, tip your head back at the sink, or stand up too quickly usually is not coming from your head. It is coming from your inner ear. The vestibular system is a set of fluid-filled canals and sensors that tell your brain where your head is in space and how fast it is moving. When loose crystals drift into a canal they do not belong in, when an illness leaves one side reporting different information than the other, or when the reflex that steadies your eyes stops keeping up with your head, the result is spinning, floating, or a world that lags a half second behind when you turn. Vestibular therapy is the physical therapy that treats that system directly. A Doctor of Physical Therapy tests which canal or which side is misbehaving, then uses repositioning maneuvers, gaze stabilization drills, and graded habituation to retrain it. It is not medication that masks the sensation while the cause sits there. It is targeted work aimed at the cause itself.

People are often surprised by how quickly this can resolve. Positional vertigo in particular can settle in one or two sessions once the right canal is identified and cleared, and it is one of the few areas of rehab where you can feel different before you leave the building. Lingering imbalance after an illness or a knock to the head takes longer, but the logic is the same. A history that gets specific

We start by pinning down what your dizziness actually does. Spinning or floating. Seconds or hours. Triggered by a position or present all day. Those details are what separate an inner ear problem from a blood pressure problem or a medication side effect. You do not need a physician referral to book this in Massachusetts, though some plans still require one for coverage, so we verify your benefits before your first visit and tell you exactly where you stand.

Testing that finds the side

Your therapist moves your head and eyes through a set of positions and watches for the specific eye movement that gives the problem away. It is quick, it is specific, and it tells us which canal or which side has stopped reporting properly. Sessions run 30 to 60 minutes, one on one with your doctor of physical therapy, and the first visit is a full hands-on evaluation rather than a questionnaire and a handout.

Treatment and a plan you can repeat

If a canal is involved, we treat it that same day with a repositioning maneuver and tell you what to expect for the next twenty four hours. If the issue is a reflex that needs retraining, you leave with gaze and habituation drills scaled to what you can tolerate today. Most patients notice improvement within about six visits, and typical care runs two to three visits a week for four to six weeks.

When a doctor comes first

Not all dizziness comes from the inner ear, and vestibular therapy is the wrong answer for some of it. Dizziness that arrives with chest pain, fainting, a sudden severe headache, double vision, slurred speech, trouble swallowing, weakness or numbness on one side, or a new loss of hearing needs medical review before physical therapy, and some of those warrant urgent care rather than an appointment. Dizziness that started with a new medication is worth a call to whoever prescribed it. Tell us what you are experiencing when you book.
