What is pelvic floor therapy?

The pelvic floor is a group of muscles at the base of your pelvis doing constant, unglamorous work. It supports your bladder and bowel, it controls when you go and when you wait, it takes part in sex, and it braces your trunk every time you lift, cough or sneeze. Like any other muscle group, it can be too weak, it can be too tight to relax, and it can lose its timing with your breathing, your abdominal wall and your hips. When that happens you get symptoms people rarely say out loud: leaking with a cough, a laugh or a run, urgency that has you mapping bathrooms, heaviness or a bulging feeling by the end of a long day, pain with sitting, tampons, exams or sex, and constipation that never quite resolves. Pelvic floor therapy is physical therapy for those muscles. Your doctor of physical therapy assesses how you breathe, how your ribs and hips move, how your abdominal wall handles load, and how the pelvic floor contracts and, just as importantly, lets go.

You do not need a physician referral to begin. Massachusetts has direct access to physical therapy, so you can book an evaluation with us directly. Some insurance plans still require a referral for coverage, so we verify your benefits before your first visit and give you your copay in writing. We accept Aetna, ASHLink, Blue Cross Blue Shield, Cigna, Fallon Health, Harvard Pilgrim, Mass General Brigham Health Plan, Medicare and Tricare, along with Workers' Compensation and auto injury claims. Flat self-pay rates are available if you are not covered.

What happens in a pelvic floor session?

Sessions run 30 to 60 minutes, one on one with your doctor of physical therapy, in a private treatment room with the door closed.

A conversation first

Your first visit starts with talking, fully clothed, about what is happening and when: bladder and bowel habits, pain, births, surgeries, what you have stopped doing and what you want back. Nothing is off limits and nothing has to be explained twice. By the end of that conversation, you and your therapist have agreed on what the examination will and will not include today.

The examination, on your terms

We look at breathing, posture, the abdominal wall, the hips and the low back, and we test strength and movement the way we would anywhere else in the body. An internal pelvic floor exam is one option among several. It is never required, it never happens without your clear consent in the moment, and declining it does not stop you from getting effective care.

Treatment and your plan

Depending on what we find, treatment may include hands-on work, breathing and pressure management, down-training for muscles that will not relax, strengthening for muscles that need it, and bladder or bowel habit changes that often help early. Most patients notice improvement within about six visits, and a typical plan of care runs two to three visits a week for four to six weeks.

Consent and privacy

You control this. Every session takes place in a private treatment room with the door closed, and you may bring a support person or ask for a chaperone at any visit. An internal exam is never required to begin care and never required to continue it, and it is never performed without your specific, informed consent on the day it is proposed. You can pause, stop or decline any part of an examination or treatment at any time, without giving a reason, and it will not change the care you receive. Your therapist explains what a technique involves, why it is being suggested and what the alternatives are before anything begins. If something is uncomfortable, saying so is enough.
